Is Myo-Inositol worth taking?

Inositol deserves considerably more mainstream recognition as a hormonal support compound. The strongest evidence is specifically in women with PCOS, where the myo-inositol and D-chiro-inositol combination has been directly compared to metformin in multiple trials with broadly equivalent outcomes for ovulation, menstrual regularity, and androgen reduction. The ratio matters: the physiological ratio of myo to D-chiro in plasma is approximately 40:1. A 2025 cell study found high-dose D-chiro-inositol alone induced a paradoxical hyperandrogenic response in granulosa cells, which is a meaningful honesty flag against using D-chiro as a standalone at high doses.

What Myo-Inositol does in the body

Insulin sensitization and downstream hormonal support, ovulation restoration and menstrual cycle regulation in PCOS, androgen reduction in hyperandrogenic women, and FSH signaling support. Myo-inositol functions as a second messenger for insulin and FSH signaling, improving cellular response to both hormones. D-chiro-inositol is produced from myo-inositol through an insulin-stimulated conversion and activates glycogen synthase, reducing the hyperinsulinemia that drives excess androgen production in PCOS.

The evidence

What the research actually shows

Moderate for PCOS applications specifically. Multiple head-to-head RCTs comparing myo-inositol to metformin have found broadly equivalent effects on ovulation rate, menstrual regularity, and androgen levels with fewer GI side effects. A 2024 umbrella review of meta-analyses confirmed meaningful benefits for metabolic and reproductive parameters in PCOS. Evidence in populations without PCOS or insulin resistance is considerably weaker. The 2025 granulosa cell finding reinforces that the combination at physiological 40:1 ratios is safer than high-dose D-chiro alone.

Food first

Getting Myo-Inositol from food

Citrus fruits, beans and legumes, cantaloupe, and whole grains contain meaningful myo-inositol from food. Reaching the 2,000 to 4,000 mg therapeutic doses from food alone is not practical without supplementation. The food-first approach to the insulin resistance underlying PCOS remains a low-glycaemic diet, resistance training, and weight management where appropriate.
