This is included for honesty rather than endorsement. Nicotine is addictive, and the line between occasional cognitive use and dependence is easy to cross for many people. A notable and frequently misunderstood finding from the COVID-19 pandemic deserves mention here: early observational data in 2020 repeatedly found lower rates of confirmed infection among current smokers, a pattern dubbed the smoker’s paradox that some attributed specifically to nicotine’s effects on ACE2 receptor expression and viral binding. However, more rigorous analysis applying causal inference criteria, including a 2025 review, concluded the evidence is insufficient to support a true causal protective effect once confounding factors, including differences in healthcare-seeking behavior, testing access, and study methodology, are properly accounted for. The COVID-19 smoker’s paradox observation, while widely reported in 2020 to 2021 literature, has since been reassessed; a 2025 analysis applying Bradford Hill causal inference criteria explicitly concluded there was insufficient evidence for a true causal association between smoking or nicotine use and reduced infection risk, underscoring how easily population-level correlational patterns can be misread as a direct pharmacological benefit.
